Two-year lookback: Your 2026 IRMAA is based on your 2024 tax return.

Cliff effect: Just $1 over a threshold triggers the full surcharge for that bracket.

Both spouses pay: If married filing jointly, both spouses pay the higher premium.

You can appeal: File Form SSA-44 if you had a life-changing event (retirement, death of spouse, divorce, loss of income).
